2012 Summer Schedule

Please print, complete, and return these forms to Eagle Lake before your arrival at camp. All forms should be turned in at least one month before the first day of your child's week.

Health History & Liability Release: This is a mandatory form. Please note, the last page must be signed by both parents of the minor attending camp. 

Parent Questionnaire: This form helps our staff get to know your child before he/she arrives.

Transportation Form: Fill this form out if you need us to pick up your child at the Colorado Springs Airport or bus station, or to authorize another individual to pick up your child from camp.

Physical Examination Form: This is a mandatory form and must be signed by a licensed medical physician.
