2010 Summer Schedule and Registration Form
Please print, complete, and return these forms to Eagle Lake before your arrival at camp. All forms should be turned in at least one month before the first day of your child's week.
Health Form: This is a mandatory form. Please print it off and have it signed by your family doctor.
Parent Questionnaire: This form helps our staff get to know your child before he/she arrives.
Transportation Form: Fill this form out if you need us to pick up your child at the Colorado Springs Airport or bus station, or to authorize another individual to pick up your child from camp.
Liability Release & Permission: This is a mandatory form.
